What to expect in a DOT Physical at Serenity Medical
Here at Serenity Medical, we believe our truckers need our full support to continue being the backbone of our nation's infrastructure.
On arrival, we'll have some paperwork for you to fill out and then we'll get your vital signs and a urine sample (this is NOT a drug test - only a health screening). Our Provider will review your health history with you, then check your vision and hearing. We'll do a full head to toe exam, repeat anything that needs to be re-checked, and talk about the certification decision with you.
In the event additional information is needed, we will work with you to obtain that information as quickly as possible. We have equipment in house to assist with the most common additional procedures that you may need (such as cleaning out the wax from your ears or checking your blood sugar), and we have laminated wallet card versions of the medical certificate for you!

Our Provider is the daughter of a trucker and has a special place in her heart for them - you will always be treated with dignity, respect, and transparency.
Most commonly requested additional documentation
Please bring your current CDL, your current medical certificate, and paperwork associated with any active waivers, variances, or exemptions.
If you have diabetes and take insulin, you MUST have a completed INSULIN TREATED DIABETES MELLITUS ASSESSMENT FORM (MCSA 5870) as well as the accompanying information as listed on the form. The form must be signed by the Provider who manages your diabetes and prescribes your insulin. Contact us if you need a copy of the form or would like a copy sent to your Provider for you!

If you have diabetes and do NOT take insulin, please bring a copy of your most recent HgA1c that is not more than 90 days old.

If you have sleep apnea, please bring a report from your CPAP machine detailing your usage. Report must be printed within the last week and have at least 30 days' usage on it.

If you have ever had a heart attack or stents, please bring a letter from your cardiologist giving clearance for you to drive commercially from a cardiac standpoint as well as the results of your most recent stress test (within the last 2 years).
